Purpose:
This document outlines the process for adding a new committee member(s) to an existing Universal Health Plan Governance Board (UHPGB) Committee. Committee members are nominated at the discretion of the committee chair(s), with input from committee members, and require approval of the Universal Health Plan Governance Board.
Process:
- Committee chair(s) and committee UHPGB members will review applications received in the past two years and select top applicants
- Staff will reach out to selected applicants to ascertain desire to serve and availability
- If prior applicant available, committee chair will present applicant to the UHPGB for approval
- If no prior applicant is available, an open application process will be initiated
- For an open application process, the committee application form will be reviewed and revised as needed
- The application will be posted to the UHPGB website
- Board and committee members will be encouraged to share the application form
- Committee chair(s) and committee UHPGB members will review the applications and provide recommendations to the Universal Health Plan Governance Board
- Board members will review and confirm new member(s)
- The board will review this policy at least annually
