Chiropractors or chiropractic assistants needing a license verification for application in another state may submit to the Oregon Board of Chiropractic Examiners a copy of the particular state's form, if one exists. Licensee must complete the top section of the form. The form may be sent to the OBCE by fax or email; refer to Contact Us above.
If no form is available, email your request to the OBCE and provide the full address of the State Board where you are applying. OBCE staff will create a license verification letter with the pertinent information, including
- Licensee's full name
- Certificate or license number
- Date of issue
- Date of expiration
- License status
- Whether any disciplinary action has been taken by this Board (Final orders are attached), or whether the licensee is in good standing
Board staff will send the completed verification by US mail to the address on the provided form, or one provided by the licensee. Five to seven business days should be allowed for processing.
Licensee may request a duplicate copy be emailed to themselves for their record. At this time there is no charge for this service.
