Consumer Summary Statements

A Consumer Summary Statement tells people about the services that are available at a community-based care facility. They also explain how residents can be required to move out if they need a higher level of care than the facility can provide.

Senate Bill 815 requires that community-based care facilities give potential residents a Consumer Summary Statement at the time of admission.
