Ordering Supplies

Convicted Offender Oral Swab Kits (for CODIS)

Convicted offender kits can be ordered from the manufacturer.  Requests for kits should be e-mailed to orders@relayresources.org. 

When ordering, please indicate how many boxes are needed.  There are 100 kits per box.  Boxes will not be split up for partial orders.  Please limit your order to two boxes or less at a time.  Be sure to include the shipping address, phone number and contact name when the order is placed.

Adult SAFE (Sexual Assault Forensic Evidence) kits can be ordered direct from the manufacturer, Portland Habilitation Center (PHC).  They are a non-profit rehabilitation facility.

Requests for kits should be emailed to orders@relayresources.org​.

When ordering, please indicate how many boxes are needed.  There are 15 kits per box, and boxes will not be split up for partial orders.  Please limit your order to less than three boxes at a time.

Child SAFE (Sexual Assault Forensic Evidence) kits can be ordered direct from the Central Point Forensic Laboratory.  Requests for kits should be emailed to lori.whitley@osp.oregon.gov.  When ordering, please indicate how many kits are needed and provide a valid UPS shipping address, (no P.O. Box).

Be sure to include the shipping address, phone number and contact name when the order is placed.

Questions pertaining to SAFE kits should still be directed to your local forensic laboratory.

​Blood and urine collection kits should be ordered via email using the OSP Stockroom Order Form (Link​).  

Only request enough items to last your agency for approximately three months.  

Please remember to include your shipping information at the bottom of the form.


Email order requests to osp.generalservices@osp.oregon.gov​.  Orders will be filled and shipped promptly unless items are backordered.
