A Community Rehabilitation Program that seeks to participate in the QRF Program must complete and submit this form.
All participating QRFs must utilize this form to submit information to DAS/SPO within 120 days of the end of their fiscal year.
Public Agencies not under DAS authority and QRFs entering into a contract must submit this form, completed with authorized signatures, the statement of work and costing workbooks to DAS for price determination.
QRFs must complete this form every two years for each employee with qualifying disabilities.

QRF Suitability Determination Notices:
THESE ARE NOTICES OF SUITABILITY FOR PRODUCTS AND SERVICES PROVIDED BY NON-PROFIT ORGANIZATIONS EMPLOYING INDIVIDUALS WITH DISABILITIES.
From ORS 279.850 (1) If any public agency intends to procure any product or service on the procurement list, that public agency shall, in accordance with rules of the Oregon Department of Administrative Services, procure such product or service, at the price established by the department, from a qualified nonprofit agency for individuals with disabilities, provided the product or service is of the appropriate specifications and is available within the period required by that public agency.
ORS 279.845 (2) requires the Department of Administrative Services (DAS) to establish and publish a Procurement List of products and services produced by qualified non-profit organizations. OAR 125-055-0020 describes the process used by DAS to determine suitability.
Newly granted suitability determinations in the QRF program:
Garten Services has requsted an increase to the size of area in the state where public agencies may purchase mailing services through the QRF program. This suitability determination will be effective on June 19, 2015.
