The Oregon Fuels Tax Group has provided these forms in Portable Document Format (PDF) for their customers. In order to view and print the PDF files, you need to install Acrobat Reader on your PC. (Some printers may require "Shrink to Fit" in printer dialog box be checked in order to print full page.)
