Everything you need to
renew an adult or child foster home is listed below.
How to renew
Prepare your documents
- Gather all required documents
before you start the online renewal application.
- You
can't save the application once you start.
- The application takes about
20-30 minutes to complete.
- To see the list of required documents:
- Refer to "Adult foster home renewal" or "Child foster home renewal" below.
- You can also view a full list of forms and documents, including some that are not required. If a document is not required, we will still process your application without it.

If you currently have individuals receiving care in your foster home, your renewal application should also include:
If you report changes on your renewal application since your last renewal cycle, we may ask for:
Make your payment (adult foster homes only)
Adult foster home payment portal
Use the adult foster home payment portal for all payments. (There are no fees for child foster homes.)

Check the fee schedule to make sure you pay the correct amount.
- Fees are nonrefundable.
- Payment is due when you submit your application.

Save your payment receipt. You will need to upload it with your application.
We will contact you
120 days before your certification expires to remind you to submit your renewal application.
We will also reach out to schedule a
renewal inspection at the home at a time that works best for you and your residents.

Request foster home changes
Read the
instructions below and use the
correct online form for the type of change you need to make.
How to request changes
Important
- Submit one change request per form.
- Gather your documents
before you start.
- You
can't save the form once you start.
- Each form takes about
15-20 minutes to complete.
- All forms are available online and can be filled out electronically. Handwritten materials are
not accepted.
- If you are increasing capacity for an adult foster home, there is a nonrefundable fee.
- Child foster homes
do not have fees.
- ODDS will review your change request.
- We may ask for more information or missing documents.

Watch your email. If we need additional information, you must return documents by the deadline provided.
- You will get a confirmation email when your changes are completed.
If you need to

- Name (include a copy of your updated Social Security card)
- Email address
- Mailing address
- Phone number
If you need to
add a co-provider or resident manager outside of your renewal cycle:

remove a provider, co-provider or resident manager at any time:

If you need to
update your business information:
Use the foster home amendment form
Use this form if you are:
- Changing the
name of your business.
- Changing
how you report to the IRS:
- Changing from a Taxpayer Identification Number (TIN) or Employer Identification Number (EIN) to the provider's Social Security number.
- Changing from the provider's Social Security number to a TIN or EIN.
