How to submit applications
The Board recommends using its online Applicant Portal for submitting your applications. This portal is secure, fast, and simple to use. Certain supporting documents such as official transcripts and work experience verifications will need to be sent outside the system on your behalf from official sources.
Other ways to submit
If you cannot use the Applicant Portal, you can download the application form, fill it out offline, save it as a PDF, and submit it through the Board's Box link. If you need an accessible form, please contact the Board office.
If you need to mail your application form, please use the mailing address listed in the Contact Information section below. Make sure your documents are addressed correctly to avoid delays.
Protecting your information
For your privacy, do not send any documents containing personal identification information by email. Email is not considered secure enough for these submissions.
