The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.  
​Oregon State Board of Nursing (OSBN) took final disciplinary action against the following nurses or nursing assistants during its November 15, 2023, Board Meeting.  The status of individual nurses or nursing assistants licensed in Oregon is available on the OSBN Verification System​​.  Or, you can download a file of all actions​​ taken during the meeting.

Jackson County
  • The Board accepted the Voluntary Surrender of Beverly K. Thigpin's practical nurse license (200330400LPN) for failing to conform to the essential standards of acceptable nursing practice.  She resides in Rogue River, Ore.
Marion County
  • The Board placed the registered nurse license of Robin L. Will (087003146RN) on probation for 12 months for failing to accurately document nursing interventions and failing to conform to the essential standards of acceptable nursing practice.  She resides in Silverton, Ore.
Multnomah County
  • The Board suspended the nursing assistant certificate of Sarah K. Amani (202207526CNA) for 90 days for demonstrated incidents of reckless behavior and engaging in abusive behavior towards a coworker.  She resides in Portland, Ore.
  • The Board reprimanded the registered nurse license of Randal S. Ralston (200842170RN) for failing to conform to the essential standards of acceptable nursing practice.  He resides in Portland, Ore.
  • The Board placed the registered nurse license of Krystal R. Sullivan (201804665RN) on probation for 24 months for using intoxicants to the extent injurious to herself or others and practicing nursing while impaired.  She resides in Portland, Ore.
Washington County
  • The Board reprimanded the registered nurse license of Sabin J. Johnson (201807180RN) for failing to properly delegate tasks of nursing care.  She resides in Beaverton, Ore.
Yamhill County
  • The Board imposed a $1,850 civil penalty against the nurse practitioner license of Teresa J. Edwards (201405956NP-PP) for practicing nursing without a current license.  She resides in McMinnville, Oregon.
Out of State
  • Connecticut—The Board imposed a $2,000 civil penalty and reprimanded the registered nurse license of Karizma M. Colon (10007784) for engaging in unsecured transmission of protected client data and violating the client's rights of privacy and confidentiality.  She resides in Ansonia, Conn.
  • Maine—The Board suspended the nursing assistant certificate of Marcelline A. Wilson (202200757CNA) for at least 14 days for failing to cooperate with the Board during an investigation.  She resides in Caribou, Maine.

  • The Board imposed a $3,500 civil penalty against the registered nurse license of Denise Damian Torj (200741579RN) for failing to keep her legal name on file with the Board at all times.  She resides in Milwuakie, Ore.
Douglas County
  • The Board placed the registered nurse license of Codey W. Fields (202106836RN) on probation for 24 months for the unauthorized removal of medications from the workplace.  He resides in Roseburg, Ore.

  • The Board placed the registered nurse license of Kaylan S. Young (201405556RN) on probation for 24 months for failing to comply with the terms and conditions of the Health Professionals’ Services Program.  She resides in Milwaukie, Ore.
Columbia County
  • The Board placed the nursing assistant and medication aide certificates of Carrie E. Benedict (000033771CMA/000033771CNA) on probation for 24 months for failing to administer medications as ordered and entering inaccurate documentation into a health record.  She resides in St. Helens, Ore.

  • The Board reprimanded the nursing assistant certificate of Tyler J. Ledford (202208838CNA) for leaving a nursing assistant assignment without properly notifying appropriate supervisory personnel.  He resides in Terrebonne, Ore.
Jackson County
