The Board of Nursing meets 12 times a year and takes disciplinary actions during each meeting.  Listed below are the meetings held for the past year.  The status and any disciplinary documents for individual nurses or nursing assistants licensed in Oregon are available on the OSBN Verification System.

Clackamas County
  • The Board accepted the Voluntary Surrender of Aaryo Bakhtiar's registered nurse and certified registered nurse anesthetist licenses (201404127RN / 202000790CRNA) for violating the terms and conditions of a Board Order.  He resides in Lake Oswego, Ore.
  • The Board reprimanded the practical nurse license of Elizabeth N. Crum (201911197LPN) for falsifying data and documenting nursing practice that didn't occur.  She resides in Lake Oswego, Ore.
Marion County
  • The Board accepted the Voluntary Surrender of Tandy R. Sholes's nursing assistant certificate (201211857CNA   ) for willful misrepresentation in applying for renewal of a certificate.  She resides in Salem, Ore.
Multnomah County
  • The Board reprimanded the psychiatric/mental health nurse practitioner license of Anna C. Long (200750076NP) with conditions for failing to maintain professional boundaries with a client and failing to conform to the essential standards of acceptable nursing practice.  She resides in Portland, Ore.

  • The Board revoked Tony D. Klein's registered nurse license (201040837RN) for being convicted of crimes with a demonstrable relationship to the practice of nursing.  He resides in Estacada, Ore.
Coos County
  • The Board reprimanded the registered nurse license of Geanna M. Berrier (200942111RN) for falsifying data and documenting nursing practice that didn't occur.  She resides in North Bend, Ore.
  • The Board reinstated the registered nurse license of Bobbie M. Felton (200942261RN) and placed it on probation for 12 months for violating the terms and conditions of a previous Board Order.  She resides in Coquille, Ore.
Deschutes County
  • The Board accepted the Voluntary Surrender of Sue A. Bassett's registered nurse and nurse practitioner licenses (084056724RN/ 084056724N6) for failing to provide documents to the Board as requested and failing to conform to the essential standards of acceptable nursing practice.  She resides in Sisters, Ore.
Jackson County
  • The Board suspended the nursing assistant certificate of Andre M. Fine (202210339CNA) for 30 days for documenting the provision of services that were not provided and entering inaccurate documentation into a client record.  He resides in Medford, Ore.

  • Washington— The Board accepted the Voluntary Surrender of Angeliki J. Themelis's registered nurse license (10001950) for practicing nursing while impaired.  She resides in Puyallup, Wash.

  • The Board revoked Candice E. Matayo's nursing assistant certificate (201802731CNA) for failing to report to the Board her conviction of a felony crime within 10 days of the conviction and failing to answer questions truthfully on an application.  She resides in Terrebonne, Ore.
  • The Board suspended the registered nurse license of Jon E. Myrold (202002915RN) for 30 days for demonstrated incidents of reckless behavior and failing to administer medications in a lawful manner.  He resides in Bend, Ore.
Jackson County
  • The Board placed the nurse practitioner license of Lauren E. Hodgdon (201809297NP-PP) on probation for 12 months for prescribing medications to herself, entering inaccurate documentation into a health record, and failing to conform to the essential standards of acceptable nursing practice.  She resides in Talent, Ore.
Jefferson County
  • The Board reprimanded the registered nurse license of Danielle S. Strobel (200841934RN) for entering inaccurate and altered documentation into a health record.  She resides in Culver, Ore.

  • California—The Board reprimanded the registered nurse license of Robert J. Driscoll (200540603RN) for failing to accurately document nursing interventions and failing to conform to the essential standards of acceptable nursing practice.  He resides in Shafter, Calif.
