1You have higher savings in the PPO plan than in the Premier plan. PPO dentists accept contracted fees as full payment for services, so you usually pay less for each visit and are protected from balance billing.  Benefits start at 80% the first plan year then increase by 10% each plan year (up to a maximum of 100%) provided the individual has visited the dentist at least once during the previous plan year.

2Individuals who enroll for coverage in this plan during an open enrollment period after they were initially eligible may have a 12 month waiting period for basic and major services and a 24 month waiting period for orthodontia. See the plan member handbook for details.
