Each vendor/partner program works a little different and things are changing quickly! Please contact them directly for the most up-to-date information.

Canopy is the Employee Assistance Program (EAP) for PEBB members (except OSU). If your employer offers this benefit, you and your eligible family members have access to confidential support through Canopy.

Employee Assistance Program (OSU employees only)

Oregon State University - Lyra Health

Lyra Health is OSU's Mental Health and Work Life services benefit. Lyra offers a full spectrum of care offerings, from preventive to severe. No matter what you're facing or where you are in your mental health journey, Lyra is here for you.

Lyra Health Benefits Comprehensive Support: Lyra Health offers a wide range of services, including therapy, coaching, and self-care tools to help you manage stress, anxiety, depression, and other mental health challenges

Easy Access: Whether you prefer in-person sessions, video calls, or digital self-care resources, Lyra Health provides flexible options to meet your needs.

Personalized Care: With Lyra, you can be matched with a provider who best fits your preferences and needs, ensuring personalized care.

On-site Provider: Employees have the option of seeing a Lyra therapist on the Corvallis campus. This therapist is available for appointments weekly on Thursdays.

  • ​To book an appointment with this provider, employees should let the scheduler know that they're looking for an 'in-person provider' and the Corvallis campus as the location.

Lyra Library: Lyra's library provides structured courses and live webinars and gatherings that help reduce stigma, support managers and teams, and build mentally healthy workplaces. Lyra Gatherings, a part of the Lyra library, are clinician-led group discussions where employees engage in structured, moderated conversations intended to strengthen the connection between social identity, mental health, and workplace experiences.

Recent Enhancements: Here are some of the ways Lyra Health has recently expanded its services to better support your well-being:

  1. One Login: Lyra Learn content, live webinars, and Lyra Gatherings are all available under one login at: osu.lyrahealth.com
  2. Enhanced Teen and Parent Coaching Programs: More specialists employed by Lyra with additional weeknight and weekend appointments available.​
  3. Provider Profile Videos: Preview personalized video introductions from Lyra providers when searching for care

A pain management program available to PEBB members (enrolled in a Providence Health medical plan) at no extra cost.

The Kaia Health app pairs clinician-designed programs with the ease of a mobile app. Using the camera in your phone or tablet, Kaia's smart technology uses motion analysis to guide you through proper form and give feedback on your movements without the need for wearables or sensors. 

To support your journey, you receive:

  • ​A personalized movement program designed by physical therapists
  • Custom relaxation and education content to help you with the mental causes of pain
  • A certified health coach to provide help and motivation along the way

As a Moda Health member, you have access to a valuable, personalized benefit to help manage and improve your health.

The Moda Health Coaching Program is available at no cost to you as part of your benefits! Health coaching takes place by phone, which makes it easy to connect from just about anywhere.

You can work one-on-one, confidentially, with a health coach to:

  • ​​Set sustainable health goals and move toward them in ways that work best for you
  • Better understand your health and how to advocate for yourself
  • Manage ongoing health issues like diabetes
  • Make healthy changes to manage diet, exercise, stress and more

Diabetes prevention for Kaiser and Providence Members

Kaiser Permanente – With Kaiser Permanente,

Moda – Moda members can access CirrusMD by logging into their Member Dashboard at www.modahealth.com/pebb or CirrusMD website at https://patients.cirrusmd.com/modahealth.

  • CirrusMD is a text-based virtual care app that lets you connect to a doctor in seconds, 24 hours a day, 7 days a week.
  • Members can also access our Nurse Advice Line by calling 800-501-5046 for help with medical issues ranging from home-care remedies to recommended emergency care at no cost.

Providence Choice and PEBB Statewide -

  • Express Care Virtual! Our licensed and board-certified providers are here to see you from 8 a.m. to 8 p.m. No appointment needed!
  • PEBB members receive a diagnosis and treatment plan, even a prescription or doctor’s note if you need one all at no cost to the member.
  • Just go to Express Care Virtual​ to get started.

  2. Once you have registered, use that login to access the links below.

All Marketplace health plans and many other plans must cover the following list of preventive services without charging​ you a copayment or coinsurance. This is true even if you haven’t met your yearly deductible.​

IMPORTANT! ​These services must be delivered by a doctor or other provider that is in-network.​


PEBB members who use tobacco have $25 deducted monthly from their pay to offset the known risks for health care costs they bring to their health plan. You can end the deduction after you quit by filling out a Mid-year Change form.

Members who use tobacco also have higher rates for optional life insurance. They can get the lower non-tobacco rate when they don't use tobacco for 12 months prior to the next Open Enrollment.

Members in PEBB Statewide, Providence Choice, or Moda plans​:

  • Call Quit for Life at 1-866-QUIT-4-LIFE (1-866-784-8454).
  • There's also a chat feature!

Members in a Kaiser Permanente plan:​​​​

  • Call the "Talk With a Health Coach" service at 503-286-6816 or 1-866-301-3866 (toll free) and select option 2.

​With your PEBB benefits, you'll have full support to quit:.​​

  • Consultations with a trained Quit Coach who knows how tough it can be to quit and ways you can stay or get back on track.
  • Materials to help you plan and stick to a personalized quit program
  • Nicotine replacement therapy
  • Coverage of certain prescribed medications to help you quit

Many have been able to quit on their own, but the chances of being successful are eight times greater with support. Whatever your reasons for wanting to quit, start your plan today!​
