2013 Health Engagement Model Program
As a 2013 HEM participant, you completed your health assessment, signed up to participate during Open Enrollment, and agreed to take and track two health actions during 2013. When you enroll for 2014 benefits, the enrollment process will include a question asking if you took and tracked two health actions during the year.

For the 2013 HEM, you choose your health actions and how you want to track them.


You have a wide variety of options for health actions:
  • For members who use tobacco: Participate in a tobacco cessation program or work with your physician on an individual plan.
  • For members whose health assessment indicates they are overweight (a BMI that exceeds 27, or waist circumference that exceeds 35 inches for a woman and 40 inches for a man): Participate in a weight management program or work with your physician on an individual plan.
  • Participate in a physical activity challenge or monitoring program. Examples include online programs available through Kaiser, a pedometer step program, or other exercise program like these sponsored by the arthritis foundation in Oregon and Southwest Washington: http://public.health.oregon.gov/DiseasesConditions/ChronicDisease/Arthritis/Documents/afprgrms.pdf.
  • Be screened in accordance with recommended U.S. Preventive Services Task Force guidelines. Examples are screening for cancer via mammogram, PAP test or colonoscopy; have a recommended immunization, physical exam or dental exam.
  • Participate in a community-based health improvement program. Example sponsors are American Heart Association, American Cancer Society, hospital groups, community colleges, extension services, or local parks or recreation centers. Options include healthy cooking and shopping classes, health education courses, and Living Well with Chronic Conditions (http://public.health.oregon.gov/diseasesconditions/chronicdisease/livingwell/Pages/Index.aspx).
  • Complete an online health education class. Examples include carrier offerings and medical and health courses from independent sources, such as those provided by the Khan Academy (http://www.khanacademy.org/#healthcare-and-medicine).

In terms of tracking, use what works best for you. Some suggestions:

  • Keep a log or journal
  • Download and use a tracking app
  • Use an online tracker
  • Mark a calendar
  • Send email to yourself
