- Visitor application form (forensics patients only) – To ensure patient safety and security, all OSH visitors must be approved in advance. Fill out this form and submit it to the Communication Center, 2600 Center St. NE, Salem. For more information, call 503-945-2800 or 800-544-7078.
- Information release – Due to privacy laws, a family member or loved one must complete the DHS|OHA form 2099, so that the hospital may share information with you and involve you in your loved one's treatment and recovery.
Policies and procedures
Everything at Oregon State Hospital is governed by policies. "Procedures" are the guidelines for following those policies. Here are some that are important for family and friends to know about. If you feel that the hospital's policies are not being followed, contact Deborah Howard, director of Consumer and Family Services.
- Patients' rights
A plain-language version of the hospital's policy that spells out every patient's rights. This document also is printed in the patient handbooks, which every patient receives upon admission.
- Respectful, timely communication by hospital staff with family and friends.
Once an information release form (2099) has been completed and approved for you, "All Oregon State Hospital staff shall communicate with family and significant others in a responsive, respectful, timely, and collaborative manner ..."
If a patient is unable to resolve an issue informally by talking with a nurse, nurse manager or other staff member, he or she can file a grievance. "Whenever possible a patient or representative shall attempt to resolve grievances informally..."
- Languages other than English.
"... patients, family members, and significant others with limited English proficiency shall have meaningful access to services, treatment, and programs in their primary language ..."
