Apply for License, Permit or Endorsement

The various pathways to licensure for dentists, dental hygienists and dental therapists appear below. Please review the following information carefully before submitting your application.  Failure to meet the requirements prior to submitting your application could result in your application being rejected.  Fees paid for incorrectly submitted applications are non-refundable and non-transferrable. Dental Licensure

General Information

​This application is intended for use by those individuals who have taken and passed a clinical examination* within the past five years.  

*Clinical Examination refers to any regional testing agency, state dental licensing authority, a national testing agency or other Board-recognized testing agency.

Application for License to Practice Dentistry
​(1) An applicant to practice general dentistry, in addition to the requirements set forth in ORS
679.060 and 679.065, shall submit to the Board satisfactory evidence of:

(a) Having graduated from a school of dentistry accredited by the Commission on Dental Accreditation of the American Dental Association; or

(b) Having graduated from a dental school located outside the United States or Canada, completion of a predoctoral dental education program of not less than two years at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association, and proficiency in the English language; and

(c) Certification of having passed the dental examination administered by the Joint Commission on National Dental Examinations or Canadian National Dental Examining Board Examination.

(2) An applicant who has not met the educational requirements for licensure may apply for examination if the Dean of an accredited school certifies the applicant will graduate.

(3) An applicant must pass a Board examination consisting of a clinical portion administered by the Board, or any clinical Board examination administered by any state, Regional testing
agency, national testing agency or other Board-recognized testing agency and a jurisprudence portion administered by the Board. Clinical examination results will be recognized by the Board for five years.

(4) A person who fails any Board approved clinical examination three times must successfully complete the remedial training recommended by the testing agency. Such remedial training must be conducted by a dental school accredited by the Commission on Dental Accreditation of the American Dental Association.

General Information
Dentists are eligible to apply for licensure without further examination if they hold an active dental license in another state, and if they have taken and passed the dental clinical examination conducted by any state or regional testing agency, in addition to meeting the requirements set forth in ORS 679.060 and 679.065.

The applicant must verify to having conducted licensed clinical practice in Oregon, other states or in the Armed Forces of the United States, the United States Public Health Service or the United States

Department of Veterans Affairs for a minimum of 3,500 hours in the five years immediately preceding application. Licensed clinical practice could include hours devoted to teaching by dentists employed by a dental education program in a CODA accredited dental school, with verification from the dean or appropriate administration of the institution documenting the length and terms of employment, the applicant's duties and responsibilities, the actual hours involved in teaching clinical dentistry, and any adverse actions or restrictions In addition, the applicant must also verify to having completed 40 hours of continuing education in accordance with 818-021-0060 within two years immediately preceding submission of their application. Dentists who have graduated from a dental program located outside the United States or Canada must also meet additional education requirements for Oregon. 

A dental license granted under 818-021-0011 will be the same as the license held in another state; i.e., if the dentist holds a general dentistry license, the Oregon Board will issue a general (unlimited) dentistry license. If the​ dentist holds a license limited to the practice of a specialty, the Oregon Board will issue a license limited to the practice of that specialty. If the dentist holds more than one license, the Oregon Board will issue a dental license which is least restrictive.

Application for License to Practice Dentistry Without Further Examination​

(1) The Oregon Board of Dentistry may grant a license without further examination to a dentist who holds a license to practice dentistry in another state or states if the dentist meets the requirements set forth in ORS 679.060 and 679.065 and submits to the Board satisfactory evidence of:

(a) Having graduated from a school of dentistry accredited by the Commission on Dental Accreditation of the American Dental Association; or

(b) Having graduated from a dental school located outside the United States or Canada, completion of a predoctoral dental education program of not less than two years at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association or completion of a postdoctoral General Dentistry Residency program of not less than two years at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association, and proficiency in the English language; and

(c) Having passed the dental clinical examination conducted by a regional testing agency, by a state dental licensing authority, by a national testing agency or other Board-recognized testing agency; and

(d) Holding an active license to practice dentistry, without restrictions, in any state; including documentation from the state dental board(s) or equivalent authority, that the applicant was issued a license to practice dentistry, without restrictions, and whether or not the licensee is, or has been, the subject of any final or pending disciplinary action; and

(e) Having conducted licensed clinical practice in Oregon, other states or in the Armed Forces of the United States, the United States Public Health Service or the United States Department of Veterans Affairs for a minimum of 3,500 hours in the five years immediately preceding application. Licensed clinical practice could include hours devoted to teaching by dentists employed by a dental education program in a CODA accredited dental school, with verification from the dean or appropriate administration of the institution documenting the length and terms of employment, the applicant's duties and responsibilities, the actual hours involved in teaching clinical dentistry, and any adverse actions or restrictions; and

(f) Having completed 40 hours of continuing education in accordance with the Board's continuing education requirements contained in these rules within the two years immediately preceding application.

(2) Applicants must pass the Board's Jurisprudence Examination.

(3) A dental license granted under this rule will be the same as the license held in another state; i.e., if the dentist holds a general dentistry license, the Oregon Board will issue a general (unlimited) dentistry license. If the dentist holds a license limited to the practice of a specialty, the Oregon Board will issue a license limited to the practice of that specialty. If the dentist holds more than one license, the Oregon Board will issue a dental license which is least restrictive.

General Information

​This application is intended for dentists who do not meet the requirements for a general license either by Licensure by Examination or Licensure Without Further Examination.  Specialty Licensure under this pathway requires taking a specialty examination administered by The Commission on Dental Competency Assessments or passing American Board examination within the immediate five (5) years prior to submitting your application.

Dental Limited Specialty

Licensure by Examination

The complete rules regarding Dental Specialty Licensure by Examination are found in OAR 818-021-0017.  In order to meet the requirements for Specialty Licensure by Examination, a dentist must have:

  1. Graduated from a CODA-accredited dental degree program.  Please note: applicants who have graduated from a dental program located outside the United States or Canada must also meet additional education requirements – see OAR 818-021-0017(2) for more information, or 
  2. Graduated from a dental school located outside the United States or Canada. 
    • ​Completed a post-graduate specialty program of at least two years at a CODA-accredited dental school.
    • ​​Proficiency in the English language., and
  • ​Either:
    • ​​​Active licensure as a general dentist in another state, obtained as a result of the passage of any clinical Board examination administered by any state, or regional testing agency, national testing agency or other Board-recognized testing agency.
    • Passed any clinical Board examination administered by any state, or regional testing agency, national testing agency or other Board-recognized testing agency., within the five (5) years immediately preceding application​, and
  1. Passed Part I & Part II of the dental examination administered by the Joint Commission on National Dental Examinations (JCNDE) or Canadian National Dental Examining Board Examination.
  2. Satisfactorily completed a CODA-accredited post-graduate specialty program.
  3. Passed, in the five (5) years immediately prior to application, a specialty examination approved by the OBD.  Please note: Dentists who passed an OBD-approved specialty examination more than five (5) years immediately prior to application do not meet the requirements for Dental Specialty Licensure by Examination; these individuals must meet the requirements and apply for Dental Specialty Licensure Without Further Examination (LWOFE).

General Information

This application is intended for dentists who do not meet the requirements for a general dental license by either Licensure by Examination or Licensure Without Further Examination. Specialty licensure under this pathway requires taking a specialty examination administered by The Commission on Dental Assessments or passing the American Board examination, clinical hours of practice and CE prior to submitting an application for a specialty license.

Application to Practice as a Specialist

(1) A dentist who wishes to practice as a specialist in Oregon, who does not have a current Oregon license, in addition to meeting the requirements set forth in ORS 679.060 and 679.065, shall submit to the Board satisfactory evidence of:

(a) Having graduated from a school of dentistry accredited by the Commission on Dental Accreditation of the American Dental Association and active licensure as a general dentist in another state. Licensure as a general dentist must have been obtained as a result of the passage of any clinical Board examination administered by any state or regional testing agency;

(b) Certification of having passed the dental examination administered by the Joint Commission on National Dental Examinations or Canadian National Dental Examining Board Examination; and

(c) Proof of satisfactory completion of a post-graduate specialty program accredited by the Commission on Dental Accreditation of the American Dental Association. 

(d) Passing the Board’s jurisprudence examination.

Dentists graduated outside the United States or Canada
(2) A dentist who graduated from a dental school located outside the United States or Canada who wishes to practice as a specialist in Oregon, who does not have a current Oregon license, in addition to meeting the requirements set forth in ORS 679.060 and 679.065, shall submit to the Board satisfactory evidence of:

(a) Completion of a post-graduate specialty program of not less than two years at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association, proficiency in the English language, and evidence of active licensure as a general dentist in another state obtained as a result of the passage of any clinical Board examination administered by any state or regional testing agency; or

(b) Completion of a post-graduate specialty program of not less than two years at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association, proficiency in the English language and certification of having successfully passed the clinical examination administered by any state or regional testing agency within the five years immediately preceding application; and

(c) Certification of having passed the dental examination administered by the Joint Commission on National Dental Examinations or Canadian National Dental Examining Board Examination;

(d) Passing the Board’s jurisprudence examination; and

(3) An applicant who meets the above requirements shall be issued a specialty license upon:

(a) Passing a specialty examination approved by the Board within the five years immediately preceding application; or

(b) Passing a specialty examination approved by the Board greater than five years prior to application; and

(A) Having conducted licensed clinical practice in the applicant’s postdoctoral dental specialty in Oregon, other states or in the Armed Forces of the United States, the United States Public Health Service or the United States Department of Veterans Affairs for a minimum of 3,500 hours in the five years immediately preceding application. Licensed clinical practice could include hours devoted to teaching the applicant’s dental specialty by dentists employed by a dental education program in a CODA-accredited dental school, with verification from the dean or appropriate administration of the institution documenting the length and terms of employment, the applicant's duties and responsibilities, the actual hours involved in teaching clinical dentistry in the specialty applicant is applying for, and any adverse actions or restrictions; and;

(B) Having completed 40 hours of continuing education in accordance with the Board's continuing education requirements contained in these rules within the two years immediately preceding application.

(4) Any applicant who does not pass the first examination for a specialty license may apply for a second and third regularly scheduled specialty examination. If the applicant fails to pass the third examination for the practice of a recognized specialty, the applicant will not be permitted to retake the particular specialty examination until he/she has attended and successfully passed a remedial program prescribed by a dental school accredited by the Commission on Dental Accreditation of the American Dental Association and approved by the Board.​

General Information

​The Board may certify a dentist as a specialist if they are licensed in Oregon as a general dentist.  Adding a specialty to a general license allows the licensee to hold themself out as a specialist, but does not limit the licensee to that specialty only.  Limiting their practice to their specialty would be up to the licensee.

Requirements to Apply

​Application for Certification as a Specialist

  • Hold a current Oregon dental license;
  • Is a diplomate of or a fellow in a specialty accredited or recognized by the American Dental Association; or
  • Has completed a post-graduate program approved by the Commission on Dental Accrediation of the American Dental Assocation; or
  • Was qualified to advertise as a specialist under former OAR 818-010-0061.

​There are no fees to add a specialty to a general license.

Dental Individual Licensure Forms

Dental Faculty Licensure

General Information

An applicant for a dental faculty license must be a full-time instructor of dentistry engaged in dental activities, including but not limited to: participation in a faculty practice plan within the scope of the applicant’s employment at Oregon Health and Science University.

Dental Faculty Licensure Application ​

An applicant for a dental faculty license must be a full-time instructor of dentistry engaged in dental activities: including but not limited to participation in a faculty practice plan within the scope of the applicant’s employment at Oregon Health and Science University and either:

​​(a) Be a graduate of an accredited dental school; or

(b) If the applicant is not a graduate of an accredited dental school, have a certificate or degree showing successful completion of an advanced dental education program of at least two years’ duration from an accredited dental school and:

(A) Be licensed to practice dentistry in another state or a Canadian province; or

(B) Have held an instructor’s or faculty license to practice dentistry in another state or a Canadian province immediately prior to becoming an instructor of dentistry at Oregon Health and Science University; or

(C) Have successfully passed any clinical examination recognized by the board for initial licensure; or

(D) Be certified by the appropriate national certifying examination body in a dental specialty recognized by the American Dental Association.

Dental Hygiene Licensure

Licensure by Examination

​This application is intended for use by those individuals who have taken and passed a clinical examinations* within the past five years.  This packet contains all the forms needed to appy for lciensure in the State or Oregon and is formatted to allow for double-sided printing.

*Clinical Examination refers to any state administered or regional examination.

Licensure by Examination Application​

​​Application for License to Practice Dental Hygiene

(1) An applicant to practice dental hygiene, in addition to the requirements set forth in ORS 680.040 and 680.050, shall submit to the Board satisfactory evidence of:

(a) Having graduated from a dental hygiene program accredited by the Commission on Dental Accreditation of the American Dental Association; or

(b) Having graduated from a dental hygiene program located outside the United States or Canada, completion of not less than one year in a program accredited by the Commission on Dental Accreditation of the American Dental association, and proficiency in the English language; and

(c) Certification of having passed the dental hygiene examination administered by the Joint Commission on National Dental Examinations or the Canadian National Dental Hygiene Certificate Examination.

(2) An applicant who has not met the educational requirements for licensure may apply if the Director of an accredited program certifies the applicant will graduate.

(3) An applicant must pass a Board examination consisting of a clinical portion administered by the Board, or any clinical Board examination administered by any state or regional testing agency and a jurisprudence portion administered by the Board. Clinical examination results will be recognized by the Board for five years.

(4) An applicant who passes the clinical portion but not the jurisprudence portion of the examination may retake the jurisprudence examination without limit on the number of times. The applicant must pass the jurisprudence portion within five years of passing the clinical portion or must retake the clinical examination.

(5) A person who fails any Board approved clinical examination three times must successfully complete the remedial training recommended by the testing agency. Such remedial training must be conducted by a dental hygiene program accredited by the Commission on Dental Accreditation of the American Dental Association.

General Information
Dental Hygienists are eligible to apply for licensure without further examination if they hold an active dental hygiene license in another state, and if they have taken and passed the dental hygiene clinical examination conducted by any state or regional testing agency, in addition to meeting the requirements set forth in ORS 680.040 and 680.050. The applicant must verify to having 3,500 hours of licensed clinical practice in the past five years in other states, in Oregon, or in the Armed Forces of the United States, the United States Public Health Service or the United States Department of Veterans Affairs.
 
Licensed clinical practice could include hours devoted to teaching by dental hygienists employed by a CODA accredited dental hygiene program with verification from the dean or appropriate administration of the institution documenting the length and terms of employment, the applicant's duties and responsibilities, the actual hours involved in teaching clinical dental hygiene, and any adverse actions or restrictions If in the military, the applicant must have a letter submitted to the Board from the commanding officer outlining duties, length of service and whether any adverse actions have been reported or taken. In addition, the applicant must also verify to having completed 24 hours of continuing education in accordance with 818-021-0070 within two years immediately preceding submission of their application.

Dental Hygienists who have graduated from a dental hygiene program located outside the United States or Canada must also meet additional education requirements for Oregon.

Application for License to Practice Dental Hygiene Without Further Examination​​​

(1) The Oregon Board of Dentistry may grant a license without further examination to a dental hygienist who holds a license to practice dental hygiene in another state or states if the dental hygienist meets the requirements set forth in ORS 680.040 and 680.050 and submits to the Board satisfactory evidence of:

(a) Having graduated from a dental hygiene program accredited by the Commission on Dental Accreditation of the American Dental Association; or

(d) Holding an active license to practice dental hygiene, without restrictions, in any state; including documentation from the state dental board(s) or equivalent authority, that the applicant was issued a license to practice dental hygiene, without restrictions, and whether or not the licensee is, or has been, the subject of any final or pending disciplinary action; and

(e) Having conducted licensed clinical practice in Oregon, in other states or in the Armed Forces of the United States, the United States Public Health Service, the United States Department of Veterans Affairs for a minimum of 3,500 hours in the five years immediately preceding application. Licensed clinical practice could include hours devoted to teaching by dental hygienists employed by a CODA accredited dental hygiene program with verification from the dean or appropriate administration of the institution documenting the length and terms of employment, the applicant's duties and responsibilities, the actual hours involved in teaching clinical dental hygiene, and any adverse actions or restrictions; and

(f) Having completed 24 hours of continuing education in accordance with the Board's continuing education requirements contained in these rules within the two years immediately preceding application.

Dental Hygiene Expanded Practice Permit

General Information

​​A licensed dental hygienist who holds a valid, unrestricted Oregon dental hygiene license and who meets the requirements of ORS 680.200 may practice as an Expanded Practice Dental Hygienist after obtaining a permit from the Board.

Application Expanded Practice Permit Pathway 1​ Application​

To obtain an Expanded Practice Permit (EPP) the licensee must complete an application and pay the application fee in addition to certifying on the application that the dental hygienist has completed at least 2,500 hours of supervised dental hygiene clinical practice, or clinical teaching hours, and also completes 40 hours of courses chosen by the applicant in clinical dental hygiene or public health sponsored by continuing education providers approved by the Board​; and provide proof to the Board of the licensee's current professional liability policy or declaration page which includes, the policy number and expiration date of the policy.​

General Information

​Expanded Practice CE Provider Application

For a provider to be approved by the Board they must complete the CE Provider Application Form, and submit it to the Oregon Board of Dentistry for approval.  Once the application is received, it will then go to the full Board at their next regulary schedule board meeting for approval.

General Information

​A licensed dental hygienist who holds a valid, unrestricted Oregon dental hygiene license and who meets the requirements of ORS 680.200 may practice as an Expanded Practice Dental Hygienist after obtaini​ng a permit from the Board. ​

Expanded Practice Permit Pathway 2 Application​

​To obtain an Expanded Practice Permit (EPP) the licensee must complete an application and pay the application fee in addition to certifying on the applciation that the dental hygienists has completed a course of study, before or after graduation from a dental hygiene program, that includes 500 hours of dental hygiene practice on patients described in ORS 680.205; and provide proof to the Board of the licensee's current professional liability policy or declaration page which includes, the policy number and expiration date of the policy.

A licensed dental hygienist who holds a valid, unrestricted Oregon dental hygiene license and who has completed a course of study approved by the board, but was unable to complete the entirety of the 500 hours during their dental hygiene degree program, may apply for the expanded practice permit if they complete the remainder of the 500 hours while under the indirect supervision of a member of the faculty of a dental program or dental hygiene program accredited by the Commission on Dental Accreditation of the American Dental Association or its successor agency. The applicant must submit a Verification of Post-Graduation Faculty form in addition to the Expanded Practice Permit Pathway 2 application.

General Information

​An expanded practice dental hyienist may render services to patients and populations pursuant to ORS 680.205 and may enter into a collaborative agreement with an Oregon licensed dentists to administer local anesthesia; administer temporary restorations without excavation; prescribe prophylactic antibiotics and nonsteroidal anti-inflammatory drugs specific in the agreement; and referral parameters.

Dental Hygiene Restorative Functions Endorsement

General Information

Restorative Functions Curriculum Template​​​​

The Oregon Board of Dentistry has approved this course curriculum template to serve as an outline for the minimal requirements of an RDH restorative course that is given by a school that is accredited by the Commission on Dental Accreditation (CODA) and it would include the following:

  • 18 hours of didactic education incorporating the objectives listed.
  • ​22 hours of laboratory skill practice on typodonts incorporating the objectives listed. 
  • 24 hours of restorative practice on patients in a clinical setting where the student places both amalgam and composite restorations. (The Board has determined that “Clinical Settings” must be at CODA accredited school locations.) 
  • Didactic and clinical course examinations sufficient to determine competency. 
  • A certificate of completion must be given to students certifying that they have successfully completed the course.

Current list of Board Approved Dental Hygiene Restorative Courses​

General Information

The Board shall issue a Restorative Functions Endorsement (RFE) to a dental hygienist who holds an unrestricted Oregon license.

  • Completion of a Board approved curriculum from a program accredited by the Commission on Dental Accreditation of the American Dental Association or other course of instruction approved by the Board, and successfully passed the Western Regional Examining Board’s Restorative Examination or other equivalent examinations approved by the Board within the last five years; or
  • If successful passage of the Western Regional Examining Board’s Restorative Examination or other equivalent examinations approved by the Board occurred over five years from the date of application, the applicant must submit verification from another state or jurisdiction where the applicant is legally authorized to perform restorative functions and certification from the supervising dentist of successful completion of at least 25 restorative procedures within the immediate five years from the date of application.

General Information

Limitation of Liability Application

​ORS 676.340 Limitations on liabilty of health practitioners providing health care services without compensation; requirements; exceptions; attorney fees; applicability.

A health care practitioner who registers pursuant to ORS 676.345 and who provides health care services without compensation is not liable for any injury, death or other loss arising out of the provision of those services, unless the injury, death or other loss results from the gross negligence of thehealth practitioner.

  • Licensee must register with the Board as a health care professioal and provide a statement as required by ORS 676.345.
  • Licensee will be responsible to meet all the requirements set for in ORS 676.345.

Volunteer Application ​

An active licensed Oregon dentist or dental hygienist who will be practicing for a supervised volunteer dental clinic, as defined in ORS 679.020(3)(e) and (f), may be granted a volunteer license ​if they complete the application.

Requirements to Apply

  • ​Licensee must register with the Board as a health care professional and provide a statement as required by ORS 676.345.
  • Licensee will be responsible to meet all the requirements set forth in ORS 676.345. 
  • Licensee must provide the health care service without compensation.
  • Licensee shall not practice dentistry or dental hygiene for remuneration in any capacity under the volunteer license.
  • Licensee must comply with all continuing education requirements for active licensed dentist or dental hygienist.
  • Licensee must agree to volunteer for a minimum of 80 hours per renewal cycle.

Licensee may surrender the volunteer license designation at anytime and request a return to an active license. The Board will grant an active license as long as all active license requirements have been met.

Out of State Volunteer License Application

General Information

Dentists or dental hygienists in another state or country and in good standing, while practicing dentistry or dental hygiene in Oregon without compensation for no more than five consecutive days in any 12-month period can apply for a temporary license.

Dental Therapy Licensure

General Information

​Licensure by Examination is intended for those applicants who have passed their clinical examination within the immediate five years preceding their application. Dental Therapists are eligible to apply for licensure by examination in Oregon within five (5) years of passage of an examination described in ORS 679.606 and/or established by the Board by rule.

Application for License to Practice Dental Therapy

​​(1) An applicant to practice dental therapy, in addition to the requirements set forth in ORS 679.603 and 679.606, shall submit to the Board satisfactory evidence of:  

(a) Having graduated from a dental therapy program accredited by the Commission on Dental Accreditation of the American Dental Association; or  

​(b) Having successfully completed or graduated from a Board-approved dental therapy education program that includes the procedures outlined in OAR 818-038-0020, and includes at least 500 hours of didactic and hands-on clinical dental therapy practice.

(2) An applicant who has not met the educational requirements for licensure may apply if the Director of an accredited program certifies the applicant will graduate.  

(3) An applicant must pass a Board examination consisting of a clinical portion administered by the Board, or any clinical Board examination administered by any state, regional testing agency, national testing agency or other Board-recognized testing agency and a jurisprudence portion administered by the Board. Clinical examination results will be recognized by the Board for five years. 

(4) A person who fails any Board approved clinical examination three times must successfully complete the remedial training recommended by the testing agency. Such remedial training must be conducted by a dental therapy program accredited by the Commission on Dental Accreditation of the American Dental Association. ​​​
