Food Handler Cards

Individuals who received a food handler card while incarcerated at the Oregon Department of Corrections (DOC) may request a replacement card by completing the form below. This process is intended for individuals who have lost, misplaced, or did not receive their original card after completing the required training.

Before submitting your request:

• You must have completed a food handler training program while in DOC custody.
• Provide accurate identifying information, including your SID number and the facility where the card was issued.
• Incomplete or incorrect information may delay processing.

What to expect:

• Requests are reviewed and processed by DOC staff.
• If your information can be verified, a replacement card will be mailed to the address you provide.
• Please allow adequate time for processing and delivery.
