Renewing your License:
Board Renewal each year is November 1—December 31 (850-030-0195)
To renew, report annual CE and print your license log into
Self-Service Portal All renewals must be submitted on or before Dec. 31, 2021 by 11:59pm.
Licensees MUST submit proof of completion of the required 32
hours of continuing education (850-040-0210.) and pay the licensure renewal fee
to receive their license. (850-030-0035)

When logged into the License Portal, click on “License Renewal," follow the steps to complete your renewal for the year!
Be prepared to update your contact information, report your continuing education (click here for CE requirements), and answer questions about your personal history since your last renewal. You'll also be redirected to the Oregon Health Authority's Workplace Data Survey. You must complete the survey before your renewal is complete. Payment by debit card or credit card.
Annual Renewal Fees:
Active License - $377 includes the PDMP fee and OHA survey fee
Inactive - $142 includes the OHA survey fee
Retired - $15
