Nursing Facility Survey Process

Nursing facility surveys, also called inspections, are conducted at least once a year to determine the extent of facility compliance with federal and state conditions and standards.


Surveyors

Surveyors from the Nursing Facility Survey Unit (NFSU) of the Office of Safety, Oversight and Quality conduct unannounced recertification and licensure surveys in all nursing homes. 


Electronic Plan of Correction (ePOC)

Oregon uses the Electronic Plan of Correction (ePOC) to document statements of deficiency and send survey plans of corrections to facilities.


Quality Indicator Survey (QIS)

In February 2011, Oregon transitioned to the use of the long-term Quality Indicator Survey (QIS) process required by the Centers for Medicare and Medicaid Services (CMS). The QIS was designed to achieve the following objectives:

  • Improve consistency and accuracy of quality of life and quality of care problem identification
  • Comprehensively review the full range of regulatory care areas

Appendix PP of the State Operations Manual outlines the minimum federal requirements for all Medicare and Medicaid-certified nursing facilities.
