The list below shows all disciplinary actions issued by the Oregon Board of Physical Therapy (OBPT) since May 2001. You can sort the list by date of notice, licensee name, or license number.
To view the public document(s) associated with a disciplinary action, click on View Licensee Record. All public documents available will be listed under the Disciplinary Actions section of each licensee's public license record.
If the license number listed is "N/A", then the individual has not ever been licensed by OBPT, but their public documents can still be found by clicking through to the License Verification.
