What to do if you believe your e-lesson record is incorrect

Employees participating in the 2012 Health Engagement Model (HEM) program, along with their spouse or domestic partner, each were scheduled to complete two health education e-lessons on their plan's website by July 15.

The plans' websites recorded completions, and the plans provided PEBB with a list of participants whose records indicate they did not complete the e-lessons as scheduled. These are the records used to determine members who would have the surcharge deduction for not completing e-lessons as scheduled.

Here's what to do if you believe you should not have been included on this list.

  1. Log back into your plan's website and navigate to the page that shows you completed two e-lessons by the scheduled date.
  2. Kaiser members: Sign on at https://healthy.kaiserpermanente.org/health/care/signon and navigate to your Kaiser Healthy Lifestyle Program page.
    Providence members: Sign on at https://myprovidence.healthtrioconnect.com/app/index.page and click on the "Self Help" link on the left hand navigation menu; then click on the "Print Certificate" link next to the conversation module you completed.
  3. Take a screen shot of the page, or save the page as a file on your computer.
  4. Print the screen shot or page file.
  5. Complete a PEBB appeal form (available at http://cms.oregon.gov/DAS/PEBB/pages/forms.aspx).
  6. Fax or mail the form and the print out of the screen shot or page file to PEBB (fax and address on form).

If these documents show that you completed two e-lessons by the deadline, PEBB staff will open an appeal file, look into the discrepancy and notify you of the results.

If these documents don't show that you completed two e-lessons by the deadline, PEBB staff will not open an appeal file and will notify you that your request for appeal was rejected.
