| Yes. You MUST enroll in a 2010 medical plan. This applies to you even if you don’t want to change your medical plan and if you want to opt out. Enroll online.

| PEBB will automatically enroll you in the employee-only tier of the Statewide Plan beginning Jan. 1, 2010.

| A | If you want to cover a dependent who will be age 19 to 24 on or before Jan. 1, 2010, you MUST certify the dependent’s eligibility for coverage according to IRS and PEBB requirements.

| A | Your dependent will lose coverage beginning Jan. 1, 2010.

| How do I certify my dependent in the 19-to-24 age range? |
| A | Online, you check a box next to the dependent’s name, and you don't have to submit a form. If you enroll with a form, you provide the required information on the enrollment form. You do not need to submit a separate form. You do not need a notarized form.

| Q | If I opt out with coverage from another state employee, do I to give my agency evidence of that coverage?
|
| A | Yes. You must give your agency evidence of the other employer group coverage within five days of enrollment. You may use a copy of the other employee’s benefit summary.

| Q | If I opt out with coverage from someone who's not a state employee, what evidence of other employer group coverage can I use?
|
| A | Examples of evidence are a letter or notice from the employer that sponsors the coverage, or a copy of your medical ID card.
