| To be eligible for no-cost participation in Weight Watchers in 2011 all principal subscribers must complete an online health assessment with their health plan. The purpose of the online health assessment is to provides individuals with information about their health risks and health status and help them develop personal health goals.

| The information you submit on your online health assessment is entirely confidential. It will not be shared with PEBB, your employer, or Weight Watchers.

| Members who are not changing medical plans during open enrollment can take the assessment beginning October 1, 2010 and have it count for 2011. Members who switch between Kaiser and the plans administered by Providence will need to wait until January 1 to take the assessment with their new 2011 medical plan.

| How often do I need to take the online health assessment?

| You only need to take the online health assessment once every 12 months. For example, if you take the online health assessment in February of 2011, you will not need to take it again until the following February. All principal subscribers must take the assessment before enrolling in their first Weight Watchers series in 2011.

| What do I do once I’ve completed the online health assessment?

| Once you’ve completed your online health assessment, you must wait approximately three weeks for your Weight Watchers eligibility to process. After three weeks, call Weight Watchers at 1-866-454-2144* and provide the customer service representative with your name and the ID number from your insurance card. They will enroll you in the program of your choice—At-Work, Local meetings, or Online.

| You no longer need to call your health plan prior to enrolling in Weight Watchers. Simply complete your online health assessment, allow 3 weeks for processing, and then call Weight Watchers at 1-866-454-2144* to enroll.

| How do I renew my no-cost participation in Weight Watchers after I have completed a 13-week series?

| Here are the steps you need to take to renew, depending on whether you are participating in meetings or online:
Meetings (both At Work and Local meetings): Show your meeting leader or receptionist your pocket guide, as proof that you have attended at least 10 of 13 meetings. Your meeting leader or receptionist will then give you a special single-use renewal certificate with a code valid for one renewal. Call Weight Watchers customer service at 1-866-454-2144* and provide your name, the ID number from your insurance card, and the code from your renewal certificate. The Weight Watchers representative will then mail you a pass for the next series of meetings.
Online: Download a copy of your Weight Tracker summary to show that you have participated online for at least 10 weeks, then cancel your online subscription. Complete the Participation Form, and submit it with your Weight Tracker summary via fax 1-888-598-7704 or e-mail. You will receive a new access code within five to seven business days. When you receive your new access code, call Weight Watchers at 1-866-454-2144* and provide them with your name, the ID number from your insurance card, and the access code. Weight Watchers will give you a 14-digit promotion code to renew your online participation.
