Applications

Application Step by Step Instructions

Do not use this application if you are already a CSWA. Any person seeking licensure in Oregon must apply. Out of state licenses do not transfer.

This is a general application for all license types. As an LCSW, LMSW, or RBSW you will only need to complete pages 1-7.  A CSWA will need to include pages 8-10. Page 13 of the application is intended to be completed by all states with which you have held licensure. This may be replaced by a verification form from the verifying state(s) if this is preferred.

Previous License Verifications

Any and all professional licenses you hold OR have ever held will need to be verified directly from the board governing the license. Please send page 13 of the application to any and all social work boards. If you have any professional licenses or certifications not related to the social work field, you may send them the Non Social Work Verification Form. Any and all verification forms must be returned to our office at:

Board of Licensed Social Workers
3218 Pringle Rd SE Ste. 240
Salem, Or 97302-6310
