Eligibility and Policy
Eligibility criteria and service types for transportation services under K-Plan:
- Eligibility: Individuals are eligible for community or Employment/Day Support Activity (DSA) transportation if they need daily living care supports either during transportation or at the destination and are receiving services through the Office of Developmental Disabilities Services (ODDS). Individuals who need transportation to get to a medical appointment must use their health insurance benefits to access this benefit. For individuals who have the Oregon Health Plan – Open Card/Fee-for-Service, use the transportation order form to request non-emergency medical transportation. For those who have the Oregon Health Plan through a local Care Coordination Organization or private insurance, this service must be accessed through the insurance carrier.
Service Types: ODDS authorizes two transportation service types: Community Transportation and To/From Work or DSA transportation. The monthly cost for all combined transportation cannot exceed $577.32 without an exception authorized by ODDS.
a. Community Transportation: The purpose of community transportation through K-Plan services is to enable eligible individuals to access their local community to engage in services necessary to meet their Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).
b. To/From Work or DSA Transportation: The purpose is to enable eligible individuals to access transportation to and from their homes to their work or DSA sites.

Transportation Elements Authorized through ODDS
1. Providers available through Community Transportation
1. Personal Support Worker (PSW) Mileage: For individuals who utilize a Personal Support Worker (PSW), the PSW can be reimbursed for the mileage incurred while providing support services that include transportation to or from local community destinations to meet their ADL/IADLs.
2. Non-Medical Community Transportation Agency: For individuals who utilize an Agency for support services that include transportation to or from local community destinations to meet their ADL/IADLs, Agencies are paid a mileage rate in addition to the support service rate.
3. Public Transit: Individuals may use available public transit providers. This is typically through a monthly transit pass. When transportation is needed less frequently, such as cases where it is intended to augment PSW supports, a daily or one-time pass option may be a more reasonable and cost-effective option.
4. Taxi Cab Services: While this is the most costly option, taxis can be authorized when all other options are not available for the individual's needs. Typically, this higher-cost service is authorized because there is no available public transit service that meets the individual's transportation support needs or because a PSW or ODDS Provider Agency is unavailable to provide transportation where and when the individual needs transportation.
2. To or From Employment and DSA Transportation:
1. Personal Support Worker (PSW) Mileage: For individuals who utilize a Personal Support Worker (PSW), the PSW can be reimbursed for the mileage incurred while providing support services that include transportation to or from their work or DSA Program.
2. Non-Medical Community Transportation Agency: For individuals who utilize an Agency for transportation to or from their work or DSA program, Agencies are paid a mileage rate as a standalone service. A Calculation Tool is available for providers of this service to aid in the appropriate billing of this service, as the miles travelled must be split among riders.
