These are the eligibility criteria and service types for transportation services under the K-Plan:
Eligibility
Individuals are eligible for Community or Employment/Day Support Activity (DSA) transportation if:
- They need daily living care supports either during transportation or at the destination, and
- They are receiving services through ODDS.
Note: For transportation to medical appointments, individuals must use their health insurance benefits.
- If you have Oregon Health Plan - Open Card/Fee-for-Service, use the transportation order form to request non-emergency medical transportation.
- If you have Oregon Health Plan through a local Care Coordination Organization or private insurance, access transportation services through your insurance carrier.
Service types
ODDS authorizes two transportation service types: Community Transportation and To/From Work or DSA transportation.

Community Transportation: enables eligible individuals to access their local community to engage in services necessary to meet their Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).

To/From Work or DSA Transportation: enables eligible individuals to access transportation to and from their homes to their work or DSA sites.
The monthly cost for all combined transportation cannot exceed $577.32 without an exception authorized by ODDS.
