The easiest and most secure way to apply is through the Board's
Applicant Portal. You can create your application, submit it, and track its progress online.
If you cannot use the portal
If you are unable to use the online portal, you can download application forms from the list below. After filling them out, please mail your completed application to the Board office, or contact the Board office to discuss other options. For your security, do not email applications. If you need an accessible form, contact the Board office.
