I/DD Licensing Applications and Forms

Apply, renew or update your license or certificate with ODDS

This is required for anyone providing services for individuals with intellectual or developmental disabilities (I/DD). Here, you’ll find the steps you need to follow and the rules for delivering safe, quality support.

Agency certification and endorsement renewal

Use the ODDS Agency and Endorsement Renewal Application to:

Important

  • The application must be completed in one session; it cannot be saved.
  • A non-refundable fee is required before submitting the application and the payment receipt must be uploaded with the form.
  • Have all required documents ready before you begin.

24-hour residential or host home license

Use the 24-Hour Residential or Host Home License Application to:

  • Apply for a new license, or
  • Renew an existing license for one residential site.

Important

  • Complete either the 24-Hour Residential License Application or the Host Home License Application—not both.
  • You must submit a separate application for each site you want to license.
  • Your agency must already be certified and endorsed for this service type before applying.
  • The form must be completed in one session; it cannot be saved.
  • Make sure you pay the required fee and include the receipt with your submission.
  • Have all required documents ready before you begin.

Request agency changes

Agencies can use the Licensing and Endorsement Amendment Form to:

  • Request changes to agency business information, endorsements and licensed sites.

Important

  • Submit only one change request per form.
  • The form must be completed in one session; it cannot be saved.
  • Some change types require a non-refundable fee and the payment receipt must be uploaded with the form.
  • Upload all required documents before submitting the form.

Below is a list of the change types you can request and the documents needed for each.

​A request to change Executive Director to another person.

Required documents:

​Change in owners of the agency including change in percentage of ownership, adding or removing owners, mergers and change in board Directors.

Changing legal business name including agency legal name or adding, changing or removing a DBA (doing business as).

​The Executive Director makes a change to part of their legal name.

​Agency dissolution means permanently closing the agency. All endorsements and licensed homes under the agency will end, and the agency will no longer be authorized to operate.

A request to change contact information for the business. This may include the email, phone number, physical address or mailing address.​

Permanent age change

Changes the age range of your license from supporting children to supporting adults, or from supporting adults to supporting children. This change is permanent and will not change back unless requested.

  • Policies an​d procedures per OAR 411-325-0355 (needed only when con​verting an adult home to a children's home)

Temporary age change

Changes the age range of your license to allow an adult to live in a children's home, or a child to live in an adult home for a designated period of time. After that period of time has ended, the license changes back to the original age range.

Required documents:

  • Safety assessments must be uploaded for each person currently living in the home, as well as for the individual who will be moving in.
  • Work with the Case Management Entity (CME) to complete these assessments. Licensing will contact the CME for their recommendation before granting approval.
  • Policies and procedures per OAR 411-325-0355 (needed only when a child is placed in an adult home)

Required documents:

  • Fire and life safety assessment
  • Health and safety assessment

Endorsement addition

Requesting to add additional endorsements that the agency doesn't already provide. Many endorsement additions come with additional required documents. Please be aware of these requirements for endorsement additions.

    • Professional Behavior Services (for each Behavior Specialist)
      • Current background check
      • Updated resume
      • Degree or transcripts
      • Current Oregon Intervention System (OIS) certification
      • Redacted FBA with invoice
      • Current mandatory abuse reporting training certificate
      • Evidence of 12 hours of ongoing training (not required in first year)
    • Direct Nursing Services (for each nurse)
      • Oregon license with certificate number
      • Current background check
      • Current mandatory abuse reporting training certificate

Endorsement closure

Requesting to remove an endorsement from the agency and no longer provide supports in that service type.

Foster home application

Use the Foster Home Application to:

  • Renew an adult (AFH) or child (CFH) foster home,
  • Add a co-applicant or residential manager, or
  • Remove a co-applicant or residential manager. 
  • Note: If you want to become a new foster provider or open a new foster home, visit the Prospective Providers page.

  • The form must be completed in one session; it cannot be saved.
  • Upload all required documents before submitting the form.

  • Completed online application (available January 2026)
  • Fee receipt (non-refundable, AFH only, CFH has zero fees)
  • Caregiver qualifications
  • Financial statement
  • Health history and physician statement
  • Floor plan
  • Rent or lease agreement (if applicable)
  • Plan of daily operations​ (AFH only)
  • Sample residency agreement
  • Background check final fitness determination
  • Orientation or pre-service training certificate
    • Adult foster home orientation
      • Required if taken on or after Jan. 1, 2026 (must submit certificate)
      • Orientations completed through county programs between July 1 and Dec. 31, 2025 are also accepted, but applicants must provide proof of completion.
    • Child foster care pre-service training

Some documents are only needed if something has changed. The application will ask questions to determine whether we need them, including

  • Medical changes for applicant, co-applicant or residentail manager
  • Financial changes for applicant or co-applicant
    • If there are chan​ges, upload a financial statement
