Apply online at the Applicant Portal. This will require you to create a portal user account the first time you use the portal. With this account, you will be able to complete your application and track the status of your application after submittal. Payment of the application fee and any subsequent registration fee can be securely made through the portal.
Continue down this page to find application forms that can be downloaded and completed offline if you are unable to apply via the online portal. Contact the Board office if you need an accessible form. Completed application forms should NOT be sent via email. Forms can be mailed to the Board office or contact the Board office to discuss other options. In this case, payment of the application fee can be made online through the "Make a Payment" option found in the menu along the top of the website, or a check or money order may be mailed to the Board office.
