SNAP Periodic Report

 Keep your SNAP benefits active

About halfway through your benefit period, we'll mail you a packet to do your Periodic Report. This is to make sure your information is up to date. Turn in the report on time to keep getting your benefits.

We'll let you know when it's time

  • We'll mail you a packet when it's time to do your Periodic Report.
  • If you have a ONE Online account, you'll also get a notification in your Message Center.
  • You don't need an interview for this report. 

 Turn in the report on time

If you do not:

  • Your benefits may stop or be late

Sign and date your form

You need to sign and date your form — even if you didn't make changes.

If you do not:

  • We can't process it
  • Your benefits may stop or be late

Double check your answers

  • Make sure everything is correct
  • Keep a copy for yourself

 Send proof if asked

  • If we need more information, we'll contact you.

How to fill out the report

Review the information on the form. Fix anything that is wrong or has changed. 

 Who lives with you

Tell us if:

  • People have moved in or out (include people you usually shop for and cook meals with)
  • This may be your spouse, children, other relatives or roommates

 Your contact information

Tell us if any of these have changed:

  • Phone number
  • Email
  • Address

Important: If your address has changed, you need to tell us your new housing/shelter and utility costs.

 Your housing/shelter costs

Tell us what you pay for:

  • Rent or mortgage
  • Mortgage interest
  • Property taxes
  • Mortgage and Homeowner's insurance
  • Homeowner's Association (HOA) fees

 Your utility costs

  • Heat and cooking fuel
  • Electricity
  • Water and sewer
  • Trash collection
  • Basic phone charges

Important: Tell us what costs you pay yourself, and if someone else helps pay.

 Your income (very important)

Tell us about all the money everyone in your home gets.

Income means money you earn from work and money you get from other sources. 

Money from work includes:

  • Work wages or hourly pay - use the amount before taxes are taken out (gross pay)
  • Salary
  • Self-employment or gig work like DoorDash, home services like housekeeping, tutoring
  • Be sure to report tips and bonuses

Other money you get includes:

  • Money you earn from your savings, retirement plans or investments
  • Social Security
  • Unemployment
  • Child support
  • Veterans' benefits

Add anything that is missing.

Important: If your monthly income changed by $125 or more from what's listed on the form, you need to provide proof.

Child support you pay

If you pay child support that was ordered by a court, tell us:

  • How much
  • How often

Important: You may need to show proof if it changed.

 Work hours (if this applies to you)

Some adults need to meet SNAP work rules. If this applies to you: 

  • Tell us if your paid or unpaid hours dropped below 20 hours a week.

How to turn in the report

Choose how to return your Periodic Report and any required proof.

 Online

If you have a ONE Online account, you can log in to ONE and complete it online.

This is often the fastest way.

 By phone

Call 800-699-9075 (Monday to Friday from 7 a.m. to 6 p.m. Pacific Time).

You can also call your local ODHS office

Relay calls are welcome.

 Paper form

You can fill out the paper form you get in the mail and send it back to us. 

You can mail or fax it, or drop it off at a local ODHS office.
