If you have SNAP food benefits, you need to complete a Periodic Report to let us know if anything changes during your certification period. ​

When to complete the Perio​​dic Report

  • ​We will mail you a packet when it's time to complete the Periodic Report.
  • In most cases, this happens halfway through your certification.
  • If you report earned income and have a 24-month certification, you need to complete a Periodic Report every 6 months​.​

If you don't complete the report, your benefits will end. You will get a letter to tell you the date y​our benefits will end.​

How to complete the Pe​riodic Report

These are the ways you can complete your Periodic Report:

  • If you have a ONE Online account, you can log in and complete the report online.
  • You can call a local office​ ​and ​complete the report over the phone.​
  • You can complete the paper form that is sent to you. You can either mail it back or drop it​ off at a local office.

​Whe​n to renew

When it's time for you to renew your benefits, we will mail you a notice or renewal packet.

  • For medical benefits including Oregon Health Plan, Member Savings Program and Long-term Services and Supports, you will get a notice to complete your renewal 90 days before the due date. Read more about renewing OHP.
  • For food, cash or child care benefits, you will get a notice 45 days before the due date.
  • If you have TA-DVS (cash support for ​domestic violence), contact your local office​ if you need to extend your benefits.

How to re​​new

  • If you have a ONE Online account​, you can log in and renew online.
  • You can renew over the phone by calling the ONE Customer Service Center at 800-699-9075.
  • You can go in to a loc​al office​.
  • You can fill out a paper application and take it to your local office or send it by mail or fax.

If you have medical, food, cash or child care benefits, you need to tell us about certain changes. This includes things like changes in income or the people who live with ​you. Each program has different requirements. Check your Notice of Eligibility for what you need to report and when.

How to report a change

These are the ways you can report ​changes:

  • Log in​ to your ONE Online account and select "Report a Change" on your dashboard.
  • Call the ONE Customer Service Center at 800-699-9075 (Monday to Friday from 7 a.m. to 6 p.m. Pacific Time).
  • Visit a local office.​
  • Send us information by mail, fax or email.​

You can submit documents by mail or F​AX, or drop them off at a local office.

How to ​get to the Message Center 

​When you enter your contact information for your ONE online account, you can sign up to receive text messages about:

  • Renewal alerts and reminders
  • Appointment alerts and reminders
  • Request for information alerts and reminders
  • Returned mail notifications

Make sure the message is from us​​

You can tell the message is from the Oregon Department of Human Services (ODHS) by looking for the five-digit code that we always send our texts from: 46982

ODHS will never ask for personal information, such as a Social Security Number, to be sent over text.

Questions about using yo​ur ​medical benefits

  • Call your Coordinated Care Organization (CCO) for ​help with appointments, finding a provider or questions about your coverage.
  • If you are not covered by a CCO and need help looking for providers, call Client Services at 800-273-0557.

Lost your OHP ID​​ card

​If you are enrolled with a CCO, contact them and request a new card. If you don't have a CCO, call Client Services at 800-273-0557.

Call the ONE Customer Service Center at 833-978-1073 if you need technical help with your ONE Online account, the ONE Online Portal or the Oregon ONE Mobile app. 

Our hours are Monday to Friday from 7 a.m. to 6 p.m. Pacific Time. We accept all relay calls.

Lost or stolen c​ard

If your card is lost or stolen, report it right away so we can freeze your benefits. 

  • If it's during business hours, call the Replacement ​Line at 855-328-6715 (Monday to Friday, 8:30 a.m. to 4:30 p.m. Pacific Time). They will ​cancel you​r card and mail you a new one. 
  • If it's a weekend or after hours, call EBT Customer Service at 888-997-4447 to cancel your card. Then call the Replacement Line during business hours to ​request a new card.

EBT Cu​​stomer Service

Call 888-997-4447​ (​24 hours a day) if you need help with things like getting a new PIN or ​checking your balance.​
