Case Management Services

Case Management services are accessed through Support Services Brokerages or Community Developmental Disabilities Programs.

Case Management activities are provided by Service Coordinators or Personal Agents including assessing needs, facilitating the delivery of services to meet those needs and monitoring the services delivered to individuals.

Individuals can choose where they would like to receive Case Management services based on services that are needed and chosen by the individual.


Adults

Support Services Brokerage Personal Agents provide case management services for adults who live in their own or family home. See Support Services Brokerages in your area.

Community Developmental Disabilities Program (CDDP) Service Coordinators provide case management services for adults who live in a variety of settings including their own or family home, adult foster homes and 24-hour residential group homes. Find a CDDP in your area.
