Case Management services are accessed through Support Services Brokerages or Community Developmental Disabilities Programs.
Case Management activities are provided by Service Coordinators or Personal Agents including assessing needs, facilitating the delivery of services to meet those needs and monitoring the services delivered to individuals.
Individuals can choose where they would like to receive Case Management services based on services that are needed and chosen by the individual.
