Parking reimbursement, health or safety concerns, ADA accommodations, complaints, training or certifications, OHCC registry or Carina, copies of paystubs or W-2, and employment or income verification. The form works on smart phones, tablets, iPads and computers.
Late/partial payment request form
Submit a request online for help with:
Late or partial payments, penalty or overdraft fees associated with payment issues, or when you have concerns about your pay.

Once submitted, OHCC will review your request and provide responses to you and the SEIU 503 Member Assistance Center.

If you don't agree with the outcome of your request, the SEIU Member Assistance Center can help you file a grievance.

Providers will need to submit a new form for each partial or late payment incident.
