OHCC Customer Relations Request Form

Use this form for help with: Parking reimbursement, ADA accommodations, complaints, training or certifications, OHCC registry or Carina, copies of paystubs or W-2, and employment or income verification. 

Late or Partial Payment Request Form

Use this form for help with: Late or partial payments, penalty or overdraft fees you got because of payment issues, or concerns about your pay.

  • OHCC will review your request and respond to you and the SEIU 503 Member Assistance Center.
  • If you don't agree with the outcome, the SEIU Member Assistance Center can help you file a grievance.
  • You need to submit a new form for each late or partial payment.

Health and Safety Report Form

Use this form to report: Any health or safety issues including unsafe or unhealthy work environments, dangerous behaviors in the workplace, threats of harm or criminal activities.

Background​ check process​

When you apply to be a provider for the first time, or when you renew your background check, it is processed by the ODHS Background Check Unit​.​​​

You need to authorize the background check and make any required disclosures. You can complete your authorization and any disclosures on a computer, tablet or smart phone.​​

How to complet​e a background check

The background check s​ystem is called ORCHARDS. Please watch t​hese videos to learn how the system​ works.

If you don't have an email address

You need to have an email address to use ORCHARDS. Here are some options for getting an email address if you don't have one:

To request reimbursement for parking costs while working for a consumer, fill out an OHCC Customer Relations Request Form. Remember to include copies of dated receipts.
