Resources for conducting the Oregon Needs Assessment (ONA)

The ONA determines a person’s level of care, support needs, and risks that affect their health and safety.

Certified assessors are trained to complete ONAs. They complete ONAs for people who are new to services. They also do ONAs for people who are already getting services:

  • For adults every five years
  • For children at ages 3, 4, 5, 8, 12, 15, and 18
  • For anyone who has a change in their needs

Services coordinators and personal agents complete yearly ONAs for people whose needs have not changed. If they identify a change, then a certified assessor conducts the ONA.

Training

For certified assessors

New ONA assessors must complete a virtual instructor-led orientation and a training process with an ODDS quality assurance trainer (QAT) that may take up to 10 weeks. After they are certified, they also complete ongoing required trainings. Here are links to the trainings and resources:

For services coordinators and personal agents

Services coordinators and personal agents are required to take a self-directed online training in Workday. They can take an ONA refresher training at any time. Here are links to these trainings and resources:
