24-Hour Supervision and Support for Ages 17 to 25
Young Adults in Transition (YAT) Residential Treatment Homes are for young adults (17 to 25 years old) who experience complex behavioral health challenges. They provide 24-hour supervision and support, focusing on helping residents develop the skills needed to manage their mental health symptoms and transition into adulthood. Skill development focuses on:
- Self-managing emotions and mental health symptoms
- Nutrition, personal hygiene, clothing care and grooming
- Managing physical or health problems as needed
- Money and household management
- Communication skills for social, health care, community resources
- Recreational and social activities
YAT RTHs also provide:
- Therapy and medication management
- Connection to other services as needed (e.g., supported employment)
YAT Admissions
If you or someone you know needs YAT services, apply directly to the YAT program. YAT programs accept applications from Oregon Department of Human Services, Child Welfare, Oregon Youth Authority, community mental health programs, acute psychiatric units, young adults and more. View the application:

Send the following to each specific program the youth is interested in:
- Completed application form
- Supporting clinical documentation: This must include recent physical health history, current medications, PPD test results and at least 6 months of psychiatric care history. See page two of the application for details.
The YAT program will reach out to learn more about whether the youth would be a good fit for the program.
