Oregon Health Authority is responsible for investigating complaints about mental health treatment facilities/homes and outpatient behavioral health services, as outlined in Oregon Administrative Rules 309-035 and
309-019.
When to Submit a Complaint
Please submit a complaint when you:
- Are not satisfied with your experience with a behavioral health care provider,
and
- Have filed a complaint with your provider, but the provider did not resolve your complaint, or
- Could not file a complaint with your provider because the provider does not have an accessible complaint process.
How to Submit a Complaint
Fill out the Official Complaint Form. When you are done, click the "Submit" button at the end of the form. A staff member will contact you.
