Agency Security Officer List – A list of Agency Security Officers authorized to request security access to financial systems (R*Stars, ADPICS, OSPS, and Datamart).

Agency Security Officer Notification Form - Required form for any changes in Agency Security Officer assignment. Must be provided within 24 hours.
Datamart Standard View Access Form - Required form for Datamart access to SFMA and OSPA tables. The agency security officer submits the form to SARS Security after completing all applicable access information. Forms with incomplete or invalid information will be returned to the ASO for correction. Must have Adobe Acrobat 8 or Adobe Reader 8.
Financial Systems Security SFMA and OSPA Request Form - Required form for all SFMA and OSPA security access requests. The agency security officer submits the form to SARS Security after completing all applicable access information. Forms with incomplete or invalid information will be returned to the ASO for correction. Must have Adobe Acrobat 8 or Adobe Reader 8.
UC78 All Agency View Access Request - Agency security officer is to complete this form and submit to SARS Security.
