Applications and forms

The following applications and forms are for businesses with a valid Prefabricated Structures Program registration.

DO NOT submit applications, forms, or fees for plans or insignias if your business does not have a valid Oregon Prefabricated Structures Program registration. Services will not be provided and fees will be refunded.

These applications and forms must be used for submitting plans, calculations, testing reports, compliance control manuals (quality control manuals), revisions, applying for Oregon insignia of compliance, and requesting inspections.

Applications or forms submitted without proper documentation or required fees will not be processed and will cause delays. Follow the instructions below for submitting applications or forms requiring payment. DO NOT alter or modify program-approved applications or forms. Altered or modified applications or forms will not be accepted.

Construction may not begin on any prefabricated building or prefabricated building component required to have an Oregon insignia of compliance and no inspections will not be performed without prior approval from the Program. If construction has already begun or a building or building components have already been constructed, there is no path for approval of that building or those building components from the Prefabricated Structures Program.

Payment Options

Payment options are limited to the following:

By mail: Mail completed applications with a check or a credit card number to the following mailing addresses:

  • For Regular USPS mail, use Building Codes Division, P.O. Box 14470, Salem, OR 97309-0404
  • For Express (UPS, FedEx, etc.) mail, use Building Codes Division, 1535 Edgewater St. NW, Salem, OR 97304

By secure fax: Submit completed applications with a credit card number to the “secure fax” number located next to the credit card box on the application. 

Disclaimer: Faxing to any other fax machine will cause delays and other fax machines may not be located in secure locations. Allow 3-5 business days for payment cashiering. Do not email applications or forms with credit card information on them.

Completed applications or forms (without payment) and a plan submittal package must be submitted electronically. Please use the Prefabricated Structures Program email: plans.prefab@dcbs.oregon.gov.
