Certification Program Consultant Contracting

This page includes templates, forms and guidance documents for certified local public agencies (LPA) conducting federally-funded consultant selection under the Oregon Department of Transportation's local agency certification program.

• The table below contains templates and forms for use by Certified LPAs on federal-aid projects. Use the search bar or filtering options to filter for templates and forms that are applicable to your project type. For example, if the project is for an A&E project and will use formal selection procedures, filter on "A&E" as the predominant services type and "Formal" as the procurement type.
• In the 'ODOT Form Required' column, items marked 'Yes' are ODOT resources, templates or forms that Certified LPAs are required to use on federal aid projects. For items marked 'No', Certified LPAs may use their own agency-specific processes or templates instead of the ODOT resource.
  
• The 'Topic' column shows which step in the consultant contracting process the template or form is used, and the 'Who Completes' column indicates whether the Certified LPA or the consultant completes the form.
