Chronic Conditions & Risk Factors Data

Includes prevalence of chronic conditions, health protective and risk factors from the adult Behavioral Risk Factors Surveillance System (BRFSS) and surveys of youth.


Below are data resources provided by the Health Promotion and Chronic Disease Section (HPCDP) of the Public Health Division.

Priority Populations

Includes data on chronic conditions, risk factors, health screenings and protective factors by race, ethnicity, sexual orientation and rural residency.

Adult Health Screenings

Includes cholesterol screening, colorectal cancer screening, diabetes screening, mammogram and others.

Student Wellness Survey

Includes state data on ease of availability, friend or parental disapproval, use and perceived risks from substances for 6th, 8th and 11th graders.

Adult Chronic Conditions

Includes arthritis, asthma, cancer, chronic obstructive pulmonary disease, coronary heart disease, diabetes, heart attack and stroke.

Adult Protective Factors

Includes physical activity, healthy weight and self-reported good or better health.

Includes state and county reports for 6th, 8th and 11th graders. Note that the Student Wellness Survey ended in 2018. There is no new data.
