We need updated information every year to make sure you still qualify for the Oregon Health Plan (OHP). We call this renewing your OHP. When it is time for you to renew, we will check to see if we have your updated information.
If we already have your information, we will renew your OHP automatically. We will send you this letter to let you know.
If you get this letter, you do not need to do anything.
If we need more information, we will send you this letter. It will have directions about how to renew and what documents you need to send. Please follow the directions in this letter.
You can renew your OHP in the following ways:
You may need to fill out the OHP Part 2 Renewal form. We mailed a copy to you, but you can also get this form in English, Spanish, Russian, Vietnamese, Simplified Chinese, Traditional Chinese, Arabic, or Somali.

If we need you to apply again, we will send you this letter with a new application. Please complete and return the application.

To make sure you get your renewal letter, please tell us when your address changes. Visit the Report Changes page to learn ways to send an address change to OHP.
We have trained community partners who can help you renew your coverage in person. It’s free. Visit OregonHealthCare.gov to find a community partner in your area.
You can also call OHP Customer Service at 800-699-9075 (for TTY call 711).
If OHP asks for a response and you do not respond to your renewal letter, you may lose your OHP coverage.
