Long-Term Care Emergency Licensure Exception Request

Oregon law (ORS 678.031(7)(f)) allows long-term care employers to request licensure exceptions for non-Oregon nurses in emergency staffing situations.  These applicants will not be placed on the emergency authorization registry; they will be notified directly by OSBN staff that they have been approved.  Employers who wish to hire nurses/nursing assistants in response to this state of emergency must complete the request form below.  These requests must be completed by the facility Chief Nursing Officer / Director of Nursing Services.  The nurse/nursing assistant requested must have an endorsement application on file in the OSBN office before being assigned work.
