Licensee lists are public information and are available upon request for a fee of $100.

You will be emailed a link to the report, and you can download the report as an Excel spreadsheet. Please note- the link will expire after seven days. Please be sure to download the spreadsheet within the seven day time period. You will need to re-purchase the list if you wish to access the list again after the link expires.

License Number
License Type (OT or OTA)
Initial Date of Licensure
License Expiration Date
Public Email Address
Mailing Address
Please note, all fields may not be available for all licensees. For example, many licensees may choose not to provide a public email address.
