Getting a flu vaccine is the best way to stop the spread of flu and protect yourself, your family and your coworkers from influenza and related complications. Everyone six months and older needs a flu vaccine every year.
Every year, flu immunizations are available from your health care provider, local pharmacy or at a PEBB clinic.
Pfizer bivalent COVID boosters will be available during the flu vaccine clinics. Per the CDC, it is safe to receive the flu shot and the COVID booster at the same time. Please read the
CDC’s recommendations about mixing/matching COVID boosters.

OSPHD.TaskRequest@dhsoha.state.or.us to reserve a 10-minute slot. In the email, please put “Schedule a Flu Shot” in the subject line and include:
- Your name
- Telephone number
- Preferred date and timeslot
- Number of people who will come in to get the shot.
- Whether you will be getting the flu shot, the Pfizer COVID booster, or both. (If you’ve already scheduled your flu shot and would like to add a COVID booster, there is no need to let us know ahead of time. Simply state that you would also like the COVID booster when you arrive.)
You will receive an email or phone call to confirm your appointment time. Please request your time by noon the day prior to your preferred day and time. Print, fill out and bring the insurance authorization form below with you to your appointment.

Gather the family
- Cooperative children as young as 9 years of age can be vaccinated with parental consent.
- Injectable quadravalent is available for anyone age 9 and older

Bring your insurance card or $30 if you are without health insurance
