Housing Stabilization Program Compliance

Program Compliance ensures that Housing Retention and Homeless Services programs meet all legal and regulatory requirements while upholding strong ethical standards. Compliance adheres to federal and state regulations, laws, policies, rules, and policies within our homelessness programs and services. The team monitors program operational performance to ensure reduce compliance risk within the distribution of funding with our contracted and agreement agencies.

Pre-Doc Monitoring Forms

Forms requirement and upload

Forms are required for each monitoring review and Self-Certification. For monitoring review, you must upload the forms that are used per program you administer within 10 days of confirming the onsite review. All Self-Certification documents are required within 10 days of the deadline for submission of the certification. Program Compliance does not accept email attachments for monitoring documents.

  • Client Served list from HMIS or other Program Tracking System 
  • List of Subrecipients program/ services they provide with OHCS Grantee funds 
  • Verification of use of coordinated entry 
  • Privacy Act From 
  • HUD Mckinney-Vento Form 
  • Release of Information form 
  • Intake form and assessment of household needs 
  • Homeless or at risk of homelessness verification 
  • Policy/form/List used for applicants have no subsequent housing options and lack sufficient resources to retain housing, but are not homeless 
  • Rent Reasonableness 
  • Calculation of Rent Utility Calculation 
  • AMI (used for monitored year(s)) 
  • FMR (used for monitored year(s)) 
  • Habitability Standards Form 
  • Lead-based Paint Form Lease Agreement 
  • Rental Assistance Agreement 
  • Verification of client income and assets 
  • List of essential emergency shelter services provided 
  • Description of Re-evaluations of eligibility 
  • Housing/Case Plans and housing assistance provided 
  • Grievance/Appeals Procedures 
  • VAWA Addendum
  • Notice of Occupancy Rights, and Transfer Plan (Emergency/Relocation) 
  • Notice on Equal Access Referrals form or policy 
  • Form or Policy on how client payments are made 
  • Form or policy for notice of program termination 
  • All additional forms to support households initial and ongoing eligibility 
  • For HSP recipients, HMIS check for HSP assistance prior to enrollment 
  • Gift Card policy Gift card documentation of use/distribution
