Pay CLIA Fees Online

How to Pay CLIA Fees Online

Starting March 1, 2026, laboratories must pay their CLIA fees online. Do not mail a check. Laboratories can pay their CLIA certification fees online, through Pay.gov, a secure platform hosted by the U.S. Treasury Department.

  1. Go to the Pay.gov main page.
  2. Enter 'CLIA Fee' in search box on top right, in the blue bar that says Pay.gov
  3. The first search result should say, 'CLIA Laboratory User Fees.'  Form Number 35.
  4. Click the Continue button for CLIA Laboratory User Fees
  5. Read the information on the next page and follow instructions to complete form and make payment.

Please be aware that Pay.gov is a Federal website and that the Laboratory Compliance Program is not responsible for its content or use. CLIA State Agency personnel cannot answer technical questions regarding Pay.gov.  If you have questions about Pay.gov, please use contact information available on that website.  Contact information can be found by clicking Help at top of the page, under Pay.gov and clicking on the last Frequently Asked Question, "Who can I contact if I need help?"

Important Note: At this time Pay.gov is the only accepted method for paying CLIA fees, using either credit card or ACH.  You must retain the confirmation of payment delivered by email from Pay.gov to obtain assistance from the CLIA State Agency regarding your payment.  Please do not attempt to use alternative payment methods.  Do not mail a check to the CLIA State Agency.  The CLIA State Agency cannot complete a vendor setup form for your accounting system.
