"Health Screen Testing" means tests performed without a physician's or clinician's order for the purpose of identifying health risks, providing health information, and referring the person being tested to medical care. (333-024-0375(2)
Any person who operates a health screen testing service must obtain a permit from the Oregon Health Authority, Public Health Division (OAR 333-024-0380)(1).
OHA may require submission of policies, procedures, and other pertinent information prior to any test performance on clients. If you have any questions or need assistance in completing the forms, please call 503-693-4125.
