OUSF Filing Requirements

Who, Where, When to File

All Oregon telecommunications companies holding a certificate, and cellphone and VoIP companies not required to have a certificate, which in aggregate will be called TSPs, must file the following documents: 
 

Worksheet OUS1 – TSP Identification

Who must file: 
TSPs
 
File deadline: 
By February 10 each year
 
Purpose:   
To establish or change service provider’s identification information
 
How to submit:

Worksheet OUS2 - Contribution

Who must file: 
TSPs
 
File deadline: 
By the 28th day of the second month following the end of each quarter (May 28, August 28, November 28 and February 28 each year)
 
Purpose: 
Identifies quarterly telecommunication revenues

Filing Revisions: 
The OUS2 Worksheet can be revised up to three years from the current quarter. If you have questions about this process, please contact the OUSF Administrator by email or call 973-581-5282.
 

Worksheet OUS3 – OUSF Distribution

Who must file:  
ILECs, CLECs receiving money from the fund on a per line basis 
 
File deadline:  
Within 40 days of the close of each month (by the 10th day of the following month)
 
Purpose:
To submit line-counts to receive financial support from the OUSF

Pay Telephone Providers

Pay telephone service providers seeking a refund of the OUSF surcharge must file an application.
 

Filing Compliance

All companies that fail to file by the listed deadlines may be subject to late payment charges. Companies holding a certificate may be subject to suspension or cancellation of the certificate of authority under OAR 860-032-0015.

Confidentiality

Information provided on the OUS2 and OUS3 worksheets is considered confidential and will be handled as such by the OUSF Administrator. Any other information maintained by the OUSF Administrator is not considered confidential.
