Reporting Requirements

The Oregon Public Utility Commission (PUC) requires telecommunication companies to submit various reports throughout the year. This is done to ensure high quality services for customers at just and reasonable rates.
 

Annual Fee Statement

Those companies that held a Certificate of Authority in Oregon for at least one day during the reporting year must complete and file an Annual Fee Statement and pay the annual $100 minimum fee. The report is due April 1 each year. The statement and instruction forms are typically available in January of the next year. Oregon Universal Service Fund (OUSF)

Certificated telecommunications service providers must submit quarterly contributions to the OUSF, which helps local telephone companies to ensure basic telephone service is available at reasonable and affordable rates in higher-cost rural areas throughout Oregon.
