Facilities Planning and Safety (FPS)

Overview

Facilities Planning and Safety (FPS) ensures that healthcare facilities in Oregon meet adopted physical environment standards. The team reviews construction plans and conducts final inspections at acute care and long-term care facilities to verify compliance with applicable regulations.

Plan review and inspection process

The plan review and inspection process is the first step for healthcare facilities licensed by the Oregon Health Authority (OHA) or the Oregon Department of Human Services (ODHS) to build or make certain modifications to existing licensed space or add new space. The process map below shows how the process works.

Project lead submits:

Plan review team reviews project using applicable built environment rules and provides comments back to project lead regarding areas on non-compliance.

When all areas of inspection non-compliance have been addressed by the project sponsor, FPS issues a Final Plan Approval.

At the conclusion of construction, the project lead submits a Notice of Substantial Completion to FPS. FPS conducts an inspection of the finished project.

A the conclusion of the inspection, and if all previous areas of non-compliance have been addressed, FPS issues a Final Project Approval. Next Step: Applicant proceeds to the health facility licensing process.

Licensed and regulated healthcare facilities

Acute care facilities

Acute care facilities are licensed and regulated by the Oregon Health Authority Health Facility Licensing and Certification Program. Before the plan review process can begin the facility may need to submit a letter of intent to determine if a Certificate of Need review is required. For additional information about which projects are subject to Certificate of Need, please visit the website. After the plan review process has concluded, facilities must contact the Health Facility Licensing & Certification Program to identify and complete any additional required licensing steps.

Assisted living facilities, residential care facilities and memory care communities

Assisted Living Facilities, Residential Care Facilities, and Memory Care Communities are licensed and regulated by the Oregon Department of Human Services Safety, Oversight and Quality Unit. Before the plan review process, the facility may need to follow the market study or letter of intent process required by ODHS. After the plan review process has concluded, facilities must contact the Safety, Oversight and Quality Unit for licensing of the long-term care facility.

Nursing homes and skilled nursing facilities

Nursing Homes and Skilled Nursing Facilities are also regulated by the Oregon Department of Human Services Safety, Oversight and Quality Unit. Before the plan review process, the facility may need to submit a letter of intent to determine if a Certificate of Need review is required. After the plan review process has concluded, facilities must contact the Safety, Oversight and Quality Unit for licensing of the long-term care facility.
