About OHP's Health Plans

OHP's local health plans are called coordinated care organizations or CCOs. Except for extra services and pharmacies, all CCOs offer the same OHP benefits. See the table below to learn about the CCOs available where you live.

Did you get a letter about CCO plan changes in 2020? If you have plan choices in your area, you can change plans any time between January 1 and March 31, 2020. If you are moving plans, the CCOs will work together to move your care.

How to Compare Plans

Find plans in your county

  • Click the Filter button in the table below.
  • Click "Counties" and type your county in the text box.
  • Click "Add Filter."

Make sure the CCO serves your ZIP code

  • If the CCO has ZIP code requirements for a county, this is listed under the county’s name.
  • If the CCO serves the whole county, it says "All ZIP codes."

Review CCO materials

  • View the CCO’s Provider Directory to search for specific providers by name, specialty or location.
  • View the CCO’s Member Handbook to learn about the services the CCO provides and how to access them.
  • To print information about a CCO, choose the printable PDF file in your language.

For the most complete and up-to-date information on a CCO plan, call the CCO's customer service number or visit the plan's website. This information was last updated October 2, 2019. Sign up to receive OHP health plan updates.

Some CCOs provide extra services.
These are items or services to support your health or manage a condition. Some extra services are available to all members, while others are provided on a case-by-case basis or by special request. You can contact each CCO to learn about a full list of services and how to access them.

American Indians, Alaska Natives, and members with Medicare have other options.
You do not have to use a CCO plan for your OHP services. If you don’t want to be in a CCO, you can call 800-273-0557 and tell us. (You will still need to choose a dental plan.)
