Accessing OVERS

Minimum requirements for using OVERS

  • A high-speed internet connection
  • Microsoft Edge is the preferred internet browser. However, any common internet browser, such as Chrome, Firefox, or Safari may be used. 

Should you be using OVERS?

If you report vital events such as birth, death or fetal death to the State of Oregon’s Center for Health Statistics, you can use the Oregon Vital Events Registration System (OVERS) to report electronically. OVERS is a secure, web-based system, so you must establish a user account before accessing it to report vital events. 

You may gain access to OVERS if you are:

  • A physician, naturopath, physician assistant, or nurse practitioner licensed in Oregon, Washington, Idaho, or California;
  • Medical clinic support staff in OR, WA, ID, or CA;
  • An Oregon Medical Examiner;
  • An Oregon licensed funeral director, a funeral director from a funeral home in an adjacent state licensed with a reciprocal agreement, or a funeral director's assistant;
  • A Birth Information Specialist from an Oregon hospital facility or licensed freestanding birthing center;
  • An Oregon licensed midwife or a non-licensed midwife registered with the State of Oregon’s Center for Health Statistics;
  • A hearing screener from an Oregon hospital facility;
  • A medical provider or support staff person at a facility that performs ITOPs (please see our ITOP registration page for more information);
  • A Facility Administrator.

To gain access to OVERS

Directions: To gain permission to access OVERS and enter death, birth, or fetal death record data, fax the documents as specified below to the Center for Health Statistics (CHS).

Everyone requesting OVERS access must submit:
 

In addition, fax the following based on your user type:

  • Birth Information Specialists* must submit 1) a letter on letterhead from your supervisor granting you permission to access the records associated with your facility and 2) the certificates for completing the Required Training Package**.
  • Birth Hearing Screeners must submit a letter on letterhead from your supervisor granting you permission to access the records associated with your facility.
  • Midwives* must submit the certificates for completing the Required Training Package**.
  • Facility Administrators must submit 1) a letter on letterhead from your supervisor granting you permission to access the records associated with your facility and 2) a completed Acknowledgment of Responsibilities Form.
  • Medical Certifiers* may work with your facility administrators to submit the enrollment form and ID.
  • Medical Examiners* must submit a letter from the office of the State Medical Examiner or District Attorney appointing you as Medical Examiner for your county.
  • Medical Certifier Staff and Medical Examiner Staff must submit a letter on letterhead from your supervisor granting you permission to access the records associated with your facility.
  • Funeral Directors* and Funeral Home Staff must submit a letter on letterhead from your supervisor granting you permission to access the records associated with your facility.

*To sign/certify vital records in Oregon, you must have a position as licensed staff in a hospital or licensed birth facility or have a valid professional license authorized in Oregon. This includes funeral homes with a reciprocal agreement with WA and medical certifiers licensed in WA, CA and ID.

  • Supporting documentation for birth information specialists and funeral directors - a letter from your supervisor granting you permission to access the records associated with your facility.
  • Supporting documentation for funeral home specialists, birth hearing screeners, and other data entry staff - a letter from your supervisor granting you permission to access the records associated with your facility, and two pieces of identification.

A Center for Health Statistics employee will notify you by email when your account is created. Upon receiving your username and password, you can start using OVERS.

  • For ITOP Reporters, please see our ITOP registration page for separate instructions.
  • For a Licensed or Traditional Midwife, you must complete the OVERS Enrollment Form for Midwives (updated 06/2022) and the Center for Health Statistics and National Trainings found on our Birth webpage. You will need to submit certificates of completion for these training with your application.
